networks. The company partners with a number of administrators to
do claims processing and customer service for its tiered network programs,
including such administrators as UMR (formerly Fiserv Health) (https://member.fiservhealthservices.com/portal)
and Meritain Health (formerly CBSA) (www.meritain.com), among others.
If you have questions about your plan, including medical eligibility, benefits or claims, you should contact your program administrator. Contact information for your program administrator is listed on the back of your medical identification card.
If you are looking to enroll in a program that uses the Patient Choice Network and have not received a medical identification card, check with your employer for the appropriate contact information.
