Enrolling
It’s easy to enroll in the Patient Choice program. Once you have reviewed your employer’s enrollment materials, just follow the simple steps below.
1. Select a Care System.
2. Choose a primary care clinic.
3. Complete your employer's enrollment forms.
1. Select a Care System.
When you enroll in the Patient Choice program, you and each of your family members select a Care System (a group of providers and facilities participating in the Patient Choice network). Each family member is free to select a different Care System.
Your Care System selection determines, among other things, the amount you contribute towards your coverage and your options for changing Care Systems throughout the year. (For more information view the Patient Choice Essentials brochure.)
To help you choose the Care System that will best meet your needs, Patient Choice
provides a variety of resources that let you compare the cost,
quality and service. One example, the Care System Comparison Guide,
includes information about Care System programs and service capabilities,
care processes, clinical outcomes and consumer survey results.
The Online Provider Directory is another resource where you can find a wealth of information such as a provider’s education and board certification status, and learn if they provide the care you need. You can also compare hospitals on their performance with specific procedures and conditions.
2. Choose a primary clinic.
In most cases, each family member will also choose a primary clinic within their selected Care System. Your primary clinic is the clinic you’ll visit for most of your general health care needs such as checkups and preventive care.
To complete enrollment, you'll need to know the primary care clinic number for your clinic selection.
The four-digit clinic number can be found in the Provider Directory,
available in print and online, or by calling the Patient Choice Helpline at 1-877-390-7632.
3. Complete your employer's enrollment forms.
Review the enrollment information provided by your employer and follow the instructions. List the Care System name and primary care clinic number for each family member's selection in the appropriate area of the enrollment form.
Once enrolled, you obtain care from providers affiliated with your selected Care System to be eligible for in-network benefits.
